Initial consultation (one hour): $400. Includes evaluation and recommendations.
Follow-Up Visit (30-60 minutes): $350/hr
Emails/Chart Reviews/Phone calls: Time dependent. Less than 10 mins. no charge, thereafter hourly rate, $300/hour, pro rata.
Payment
All visits must be prepaid by Paypal or check.
Cancellation Policy
Appointments must be cancelled at least 24 hours in advance.
If an appointment is cancelled with less than 24-hour notice, there will be no charge the first time; after that, the normal fee will be charged. If an appointment is forgotten, there will be no charge the first time; after that, the normal fee will be charged.
